* Please tell us what you are looking for Our practice is set up as a membership program, similar to a health club. This enables our physicians to give you the time you need to get the best care. Hurried care is not usually the best care and at Qliance we take our time. This offering is for one month of services of the following: Qliance Level 1 with REMOTE hospital coordination Provider Type: Family or Internal Medicine MD or ARNP Hospital Care: Remote coordination and supervision at any hospital and coordination with your specialty care clinician by your Qliance clinician. After Hours Coverage: Shared by all Level 1 clinicians. Qliance Level 2 with BEDSIDE hospital coordination Provider Type: Family or Internal Medicine MD Hospital Care: On-site rounds by your Level 2 clincian at a specified hospital to inform and coordinate your care provided by local hospitalists and specialists. After Hours Coverage: Personal on weekdays & shared by Level 2 clinicians on weekends. Registration Fee We charge a one time registration fee of $99 per family which will be charged at the time of registration. Should a family member enroll at a later date, we will waive their registration fee. Payment We accept credit cards, debit cards and automatic funds transfers for your annual, semi-annual, quarterly or monthly care fee. We accept credit cards, debit cards, cash and check for any incidental fees insurred during your visit. Incidental Fees Your monthly care fee covers the primary and preventive care services described on the Services page of our website. However, at times your care may require third-party services that are not included in your care fee. You may be asked to pay additional fees for items such as prescription medications, laboratory tests and outside services. In all cases, incidental items are charged at or near our cost and their prices and relevance to your care are fully discussed with you before you are charged. If you have health insurance, you may choose to have your lab tests billed to your carrier as well as have prescriptions sent to a pharmacy that accepts your plan. You can also find these services here on FairCareMD. Contract At Qliance, there is no long-term contract. If you're dissatisfied with your care, you may cancel your membership at anytime and we will prorate that month's care fee. Thank you in advance for submitting this request in writing.
