Premium Smart Care Plan  (Limited to 100 enrollees.) You pay a monthly fee and then when you are seen, you pay a reduced office visit charge and that fee includes basic labs/diagnostic tests*.

You also have direct access to your provider’s email address and cell phone 24 hours a day, 7 days a week.

Have you ever been sick or had a sick child after hours and wish you had someone you could call?

Want the convenience and privacy of housecalls any time of the day or night and even on weekends? Just want someone to ask to know if an emergency room visit is necessary?

Also, your provider will assist with emergency hospital admittance after hours. Reduced housecall fee applies.

Cost is $99 per month and $40 per visit with most basic labs/diagnostic tests included*.

(Pay 1 year in full - $1188 and receive a Comprehensive Screening Exam worth $275 free**)

Reduced housecall fee of $200 + procedures (basic labs/diagnostic tests are included in housecall fee*)
